Disability Database for children and young people
The London Borough of Merton's Disability Database is a voluntary database for children and young people aged 0 to 18 years who have a disability or special educational need.
What is the database for?
Details provided when you apply for your child to be added to the database will be used to:

Provide you with information on services relevant to children and young people with SEND where you request this when applying or when you update your entry on the database

Help inform the Local Authority’s commissioning decisions and support key partners (e.g. NHS) to understand the needs and demands for services in the borough
What are the benefits of joining the database?
The benefits of joining the database are:
- The option to receive regular updates on services and other information relevant to children and young people with SEN and disabilities
- The ability to apply for a Merton M-Card if your child/young person meets the eligibility criteria
- The opportunity to have a voice as part of any future Local Authority consultations relevant to children or young people with disabilities and or SEN

How do I apply for my child to join the Disability Database?
To join the Disability Database, you need to create an account with the Merton Local Directories.
Click on My Account at the top of this website page, select Create an Account and follow the instructions on screen to register and create a password. Once you have created your account, you can login and select the Disability Database option in your account and complete and submit your application.
As you are completing your application, you will have the the option to agree to receive a regular newsletter and you can also apply for an M-Card if your child is eligible for one. The M-Card provides a quick, easy and discrete method to raise awareness that the card holder has a disability and will be able to receive concessions from some services.
Once you have an entry in the disability database for your child / young person, you can login and amend your details at any time.
