Preparation for adulthood pathways

Young people approaching adulthood and their families may want an overview of the choices available as they prepare for adult life and learn where can find helpful support and guidance. 

The Merton Transition Team provide advice, assessment, planning and support. They aim to help make the transition into adulthood for young people as easy and smooth as it can be. 

We have developed guides for young people with special educational needs (SEN) or  disability at all levels of need both with and without an Education, Health and Care Plan (EHCP).

The guides should help you to know which services to look for elsewhere on Merton’s SEN Local Offer.
