Youth Empowerment Skills (YES)

The Youth Empowerment Skills programme helps young people (age 14-19) with Type 1 diabetes become more confident in managing their diabetes and acts as a peer support environment as they meet other young adults living with their condition.

The Youth Empowerment Skills (YES) programme combines social activities organised by a trained Youth Worker with a three-day interactive workshop delivered by the diabetes department at Guy’s and St Thomas’ hospital.

Participants will get the chance to take part in activities and discussion sessions in the six months prior to the workshops which are scheduled for August. The feedback from people taking part has been really positive.

  • Share hints and tips;
  • Where to get advice;
  • Fully funded social activities.

Referral Details

To take part in YES you need to be registered with a GP in south London. (This covers the boroughs of Bexley, Bromley, Croydon, Greenwich, Kingston, Lambeth, Lewisham, Merton, Richmond, Southwark, Sutton and Wandsworth).

You must be referred by either your GP or a member of the team at your diabetes clinic.

We accept referrals from healthcare professionals in primary, community or secondary care in any of the 12 south London Boroughs. The young person must be registered with a south London GP.

Once you’ve received consent, download our referral form from gstt.nhs.uk/YES and send the completed form to gst-tr.yes.referrals@nhs.net
