Expert Patients Programme
The Expert Patients Programme is a free, self-management course that supports people living with, or caring for someone with, one or more long-term health conditions. Common long-term conditions include: diabetes, back pain, high blood pressure, depression, anxiety, asthma, arthritis and chronic obstructive pulmonary disease (COPD).
The course offers a tool-kit of techniques to enable you to manage your condition better on a daily basis, by increasing your confidence and quality of life. Each course consists of 12 to 16 people, led by tutors who have long-term conditions

To attend a course you must either live, work or be registered with a GP in Merton. Anyone living with (or caring for someone with) one or more long-term health conditions can attend a course. The course runs once a week for six consecutive weeks. Each session lasts for two and a half hours.
