Adult Social Care Learning Disabilities (Transitions Team)
This service supports young people with learning disabilities between the ages of 18 and 25 although it assesses young people with complex needs from age 14 onwards. Please see separate guidance on transitions from children to adult social care services.
Our team includes two dedicated social workers and we are able to draw on the expertise and services of the wider Learning Disabilities team, if required. For example, we may involve a specialist learning disability nurse, clinical psychologist, speech and language therapist or occupational therapist, depending on a young person’s needs.
If eligible, we are able to provide a range of services including:
- Help to plan for the young person's future when they leave school
- Information about community-based support and activities
- Support available in the home
- Personal care and behaviour support for families experiencing difficulties managing behaviour in the home
- Respite services for parents and either at home or in a different setting
- Direct payments for young people (or others on their behalf) to purchase care services
- Overnight respite provision for young people
- Safeguarding, mental capacity assessments and deprivation of liberty safeguards
- Supported living or specialist residential placements
- Carer’s assessments
- Making sure that the young person and their parents'/carers' views are heard
- Referrals to benefits advisors, housing, the Merton Employment Team and others

Our assessments are carried out in accordance with the Care Act 2014. Links to the statutory guidance and other information about this can be found below.
You are eligible for our services if you are assessed as having ‘eligible’ care needs as defined in the Care and Support Statutory Guidance (see link below), you have no alternative way of meeting these needs and therefore wish the Local Authority to provide support, and you meet ordinary residence criteria (also defined in the Statutory Guidance).
We will assess children or young people who are already receiving services from the Children with Disabilities Team or young people are likely to have needs when they turn 18 for whom there will be a significant benefit to the individual in doing so. To establish your needs, we will ask you to complete a self-assessment questionnaire (SAQ) which you can complete with the help of parents, carers or other professionals

In order to receive support from the Merton Transition Team, the young person will need to be a resident of the London Borough of Merton and meet eligibility criteria for either children's or adult social care, dependent on age (14 to 18 for children's social care, 18 to 25 for adult social care).
